Many courses have prerequisites or corequisites that are required because specific knowledge or skills are needed to succeed in the course. Use this form along with an unofficial or official transcript to meet course prerequisites with coursework taken at another institution.

You must submit the prerequisite verification request form and transcript together, or your documents will not be processed.

Transcript requirements

Your transcript must include your name, the school’s name and the course being used to meet the prerequisite. If you wish to meet a prerequisite with a current course not yet on your transcript, please include proof of your enrollment (for example, a letter from your instructor or a copy of your current class schedule). To avoid being dropped from your CLC class, you must send the Registrar and Records office an updated transcript showing the completion of this course, and your grade, no later than the Monday prior to the start of the term at CLC.
