Make a Report 

EMERGENCIES: If your concern may cause imminent harm or is an ongoing threat to an individual or the campus community, contact the College of Lake County (CLC) Police Department at (847) 543-2081 or 5555 from a campus phone or your local law enforcement agency by dialing 911.

Please read the following categories and make a selection at the bottom of this page. 

  • Academic Concerns- For students who want to report about their grade, instructor, classroom environment, or other academic issues.
  • Academic Dishonesty Violation Form- For faculty reporting about a student's academic dishonesty.
  • Concern for a Student- For anyone reporting about a student's well-being (mental health, basic needs, wellness), or inappropriate, disruptive, and/or threatening behavior that creates an unwelcoming or hostile environment.
  • COVID-19 Concern- For any COVID related concerns or questions. 
  • Discrimination and Harassment- For anyone with concerns about being a target of, or who witnesses, discrimination or harassment. This includes but is not limited to microaggressions, stereotyping, or harmful assumptions against marginalized identities. 
  • Relationship or Interpersonal Violence, or Sexual Misconduct- For anyone with concerns about dating or domestic violence, sexual harassment or misconduct, and stalking.
